What should you do if you believe you have suffered an injury or illness as a result of your job?
- Contact us. We will represent you and be with you through every step of the legal process.
- Report the injury immediately to your supervisor. Within one working day after receiving notice or knowledge of your injury, your supervisor will give you a claim form for you to complete and return to him/her. The supervisor will give you a dated and signed copy of the claim form and will forward a copy to the workers’ compensation insurance carrier.
- Contact your doctor to find out if a report was sent to the workers’ compensation insurance carrier or your self-insured employer.
- If you encounter any difficulty or delay with the above, contact us as soon as possible.
